FAQ

• If this is a new prescription, you are usually given enough for 3 months, and then the provider will want to see you for an office visit to evaluate how well you are tolerating the new birth control. At this visit she will renew your prescription through your next annual exam.
• You may be due for your annual exam. If so, schedule your appointment and ask the pharmacy to request a 1 month refill only. We can only give a 1 month extension as 12 refills are given at the time of the annual.

• Some patients on hormones must be seen and monitored every 6 months. If you are due for this office visit then you must make an appointment to obtain refills.
• If you are up to date with your appointments, ask the pharmacy to send a refill request to the office.

• Ask your current pharmacy to transfer the information to the new pharmacy.

• There is a $15 fee for lost prescriptions. Call the office at 407-898-1500 ext. 13 and leave your pharmacy and prescription information as well as your credit card information and we will call the prescription in and charge your credit card for the $15 fee.

• Ask the mail-away pharmacy to fax a refill request. This can only be done if you are up to date with your well woman exam and/or follow-up appointments.
• If you are due for an appointment but need a refill to get you until the appointment we can fill 1 month for you at a local pharmacy. Ask the pharmacy to request 1 refill or fill out a prescription refill request online with a local pharmacy’s information.
• At your exams, remind your provider that you use a mail-away pharmacy so they make sure to write the prescription correctly for you to mail in or make sure to bring any forms and we can fax it for you if they are filled out.

• Yes, we are currently accepting new patients. Please fill out an online request for an appointment or call our office at 407-898-1500 ext. 10.

• Yes, we require appointments.

• No, problems that you are having may not be able to be addressed at your annual well woman visit.
• We prefer to address any problems before we do your annual well woman exam.
• Insurance benefits are often different for well/preventative vs. sick/problem visits and we must comply with your insurance company contracts or the claim will be denied.
• If you have multiple concerns, please understand that your provider may need to ask you to come back in order to complete your annual exam and/or to address your problem(s) thoroughly.

• The provider will go into detail with you about the results and make sure that you understand what your results mean and answer any questions.

• We want to offer you the first openings we have so you can get your results quick, and the Nurse Practitioner’s schedule enables us to get you in quicker.
• When your results come in, the provider who performed or requested the testing will review your results before your follow-up appointment as well as note any follow-up plans.
• The nurse practitioner will then be discussing with you what the provider noted and recommends.

• Yes. If your appointment is set for over one month we will send a postcard reminder.
• Everyone will get a call 1-2 business days before the appointment by our automated system.

• The appointments are set for every 6 months to make sure dosages do not need to be changed and that we monitor your blood pressure and any possible side effects.

• Yes, you must come in for an appointment to be evaluated by a provider. Otherwise you can see your Primary Care Physician or go to a walk-in clinic.

• If you are having abnormal bleeding (that is NOT breakthrough bleeding due to a new birth control start) you must make an appointment to be evaluated by a provider.

• For a change or start of birth control, you need to make an appointment. At that time we will discuss, evaluate, and choose the best option for you.

• The consent that you were given when you started birth control, found here, will give you some general information.
• You may also refer to the insert that came with your birth control or go to the manufacturer’s website for more information regarding your specific prescription.

• No, we do not do any pregnancy care. We will run a pregnancy test on an established patient only. Then we will refer you to an obstetrician if needed.

• You must fill out a records request form. This is available on the website here and can be mailed or faxed to the office. It may take up to 7-10 business days to complete your request.

• If your records are being sent to another physician, there will be no charge.
• If you are requesting your records for yourself, these records may incur a charge, depending on the number of pages. We send 5 pages for free, and it is $1 per page for each additional page.

• This means that you scheduled an appointment which you did not show to the office for.

• If it was a regular office visit or annual exam: $35
• If it was a procedure (including in-office procedures, Ultrasounds, Urodynamic testing, or Pelvic Floor Muscle Therapy): $50

There are multiple ways to pay off a balance that you may have with our office, including those for records and no show appointments.

• Come into the office and pay with cash, check, or credit card.
• Mail a check in to the office.
• Call the office at 407-898-1500 ext. 11 with your credit card information to be charged.
• Return any statement you receive in the mail with your credit card information or a check.
